Helpful Documents to Have Available

You do not need to have every document ready before reaching out. However, if you are seeking insurance-funded ABA services, it may be helpful to have a copy of your child's diagnostic evaluation, insurance card, and any recent treatment or school-related reports.

For ABA insurance authorization, many payers require a formal Autism Spectrum Disorder diagnosis (F84.0) and documentation supporting medical necessity. If you are unsure whether your child's evaluation includes the needed information, you can still submit an inquiry and we will help clarify next steps.

Interested in School Support?

If you are seeking school-based observation, consultation, or collaboration, please mention this in your inquiry. School-based services are reviewed case by case and may depend on clinical need, insurance requirements, caregiver consent, school approval, and whether outside providers are permitted on school grounds.

You do not need to have school permission before reaching out. We can help clarify what information may be needed before school-based support can be considered.

Secure Inquiry Form

This form is completed through SimplePractice, a HIPAA-compliant platform used for healthcare communication and intake. Information submitted through this form helps us understand your inquiry and determine appropriate next steps.

Completing this form does not guarantee services or establish a provider-client relationship. A formal service relationship begins only after appropriate intake, consent, documentation, and service agreement processes are completed.
